Some people get hit by a truck and their deaths are easy to classify
But some people get hit by a truck, then spend a month in a coma after which they develop permanent kidney damage due to mechanical ventilation. They then slowly decline, the transplant doesn't take

What's the cause of death? Did they die with kidney disease or from it?

If a healthy 20 year old dies from a heart attack, it's easy to code. If the same heart attack happens to a 90-year-old person with diabetes, lung cancer, and chronic heart failure, it's far more complicated
