Chronic pelvic pain in a young lady may not always be infection, what else can it be?

She came to us with complaint of this excruciating lower abdominal pain on both sides that has refused to leave her for about 14 months.
She had done abdominal scan and series of lab tests and they told her she had tubo-ovarian abscess.
She's had more than 6 full courses of different antibiotic regimens all to no avail. Her fiance recently broke up with her as a result because to him, she was cheating and that's how she got infected.

She was totally fed up but in all these, she'd never seen a specialist.
After evaluation, turned out she had 'chocolate cysts', a form of endometriosis.
Endometriosis is when one has endometrial tissue (the inner part of the womb women shed monthly) lying outside the womb,Commonly inside the abdomen,bladder, navel,ovaries etc.
When they're found in the ovaries it's called Endometrioma/Chocolate cyst.
About 5-10% of young women have one form of endometriosis.
Cause is unknown for now but back flow of menstrual blood through the fallopian tubes is the most common accepted theory. This is why some
School of thought discourage sex during menses.

Symptoms include abdominal pains that worsens during menstruation, pain while pooping or shagging. Also cyclical bleeding from the nose, navel or anus depending on the location of the tissue.
The most important complication of this condition is infertility.
This accumulation of altered blood causes inflammation and scarring of the fallopian tube making it impossible for the patient to conceive without assistance. Hence early diagnosis and treatment is important.
In conclusion, the need to consult your doctor whenever you notice something unusual in your body cannot be overemphasized. It saves you money, time and limits complications.
