If they are ‘Awareness’ actions then surely you don’t need to improve?
Using language like RAISE, DISCUSS, HIGHLIGHT, INFORM, REMIND may be useful and will certainly raise awareness regarding your results or findings. BUT is highly unlikely to CHANGE anything.

We suggest you use a variety of action types to create an effective and robust action plan.

Awareness (I believe) is always a solid first step. If teams are not aware of the data/results/findings/recommendations then how can they be engaged to CHANGE.
Learning may also play a part in improving and change.
BUT HOW DO YOU KNOW THAT IS THE PROBLEM?

Administrative actions are nearly always necessary. Documenting the findings, sharing, adding to agendas, updating forms.
Evidence of your actions and the completion of them is very important.

Escalation may be one of the first things you need to do. If your findings have highlighted a risk then that needs escalating via a register or perhaps by raising within governance forums.

Investigation!
Are you confident you KNOW what is impacting your results. HOW?
You MAY need to look into starting a QI project, you MAY need to collect additional data. Investigative actions are extremely important, they show you are taking a robust approach to improvement.

Change actions are often what we DO that will ultimately being sustained change.
They may be embedded into other actions buts it important they are within your action plan.

Look at your action plan and ask...
Have we shared the results?
Do we require additional learning/teaching?
Is all documentation up to date?
Does anything need escalating?
DO WE KNOW WHAT THE PROBLEM IS? HOW DO WE KNOW THIS?
What have we changed?






Is really is worth finding time to do create an effective action plan.
We often get surprised when we see no change when all we are doing is raising awareness, or changing documentation, or escalating.
This is why an investigative type action is REALLY IMPORTANT.
